Tips for Completing Your Forms

Use a separate form for each child.
Fill out all parts highlighted in blue and white.
Review any changes with your center director.
Bring your enrollment form and driver’s license or ID so your center director can check your information.

If any of your information changes later, meet with your center director. They’ll review the updates, check your payment amount, sign the new form, and keep a copy in your family file.
